NHS South West London has been working with Carshalton Fields Surgery to find an urgent solution after the owners of the practice building on Crichton Road served notice, which means the practice needs to move.
Carshalton Fields Surgery and Wallington Family Practice have submitted a business case to merge, which NHS South West London has approved.
Carshalton Fields Surgery on Crichton Road will close on Friday 28 June 2024.
There are a range of benefits for patients at both Carshalton Fields Surgery and Wallington Family Practice:
- Jubilee Health Centre is a purpose-built health centre offering significantly improved premises for Carshalton Fields patients.
- Carshalton Fields Surgery patients will also benefit from a greater range of services, clinical staff, and local services that operate at Jubilee Health Centre, including radiography, Musculoskeletal Health (MSK), and Phlebotomy.
- All the staff and clinicians at Carshalton Fields Surgery will be retained and transferred to Wallington Family Practice – this will bolster the service Wallington Family Practice is able to provide.
- Carshalton Fields is a CQC rated Good practice and can support Wallington Family Practice make improvements to their processes and services.
Carshalton Fields Surgery’s current building is privately owned and their landlords have the legal right to pursue a private sale of the premises. Unfortunately, under the lease agreement the practice holds, NHS South West London has no legal route to challenge the sale of the building.
Patients at Carshalton Fields will be automatically registered at Wallington Family and their medical records will automatically transfer on 1 July 2024, so they don’t need to take any action.
All patients registered at the practice will receive a letter outlining the changes and with information about how they can ask questions and discuss any concerns they have.
Details of an event in May to discuss the merger will be promoted soon via the practice website and on posters within practice.
Practice staff will also ensure that all vulnerable and housebound patients are supported during this change.

Frequently asked questions
Why are the practices merging and the Carshalton Fields Surgery premises closing?
Carshalton Fields Surgery on Crichton Road is owned by a private property owner, and the lease has expired. The property owners have the legal right to pursue a private sale of the premises, and we have no legal powers to intervene.
This has been a very difficult situation, and we are committed to ensuring that patients registered at the surgery, continue to receive the services they need. We know that Carshalton Fields Surgery is well respected by their patients, practice team, as well as our NHS South West London team.
To support the surgery, we have met with the practice manager to discuss the options available. When considering the options available, the merger provides the best solution and minimal impact for both patients and staff.
The staff at both practices are very enthusiastic at the decision to work together and combine services, and keen to further improve access, experience, and the available services for their patients.
What will the merger of the two practices and the closure of Carshalton Fields Surgery involve, and how would this affect me?
Registered patients do not need to take any action and the change will not affect your registration. For those patients registered with Carshalton Fields Surgery the patient records will transfer over to the Wallington Family Practice once the contracts have merged.
The practices will work with the primary care team at NHS South West London to identify and support vulnerable and housebound patients with this change – please contact your practice if you need this support.
