Perinatal pelvic health survey

Take our survey to help us make sure that everyone who uses midwifery services in South West London has the right access to education, support and information about pelvic health before, during and after pregnancy.

We appreciate that this can be a very sensitive and private topic of conversation, and we would like to reassure you that all the information submitted in this survey is anonymous and will be used in the strictest of confidence.

Any information you feel comfortable in providing will be used for the purposes of improving the level of service and support we can offer all women with pelvic health complications.

We would like to hear the views of anyone who has given birth in the last two years and those who are currently pregnant.
