Stonecot Surgery has been shortlisted for Primary Care Provider of the Year at the 2026 HSJ Awards, recognising its work to improve care for patients with long-term health conditions.
More than one in three patients at the surgery has a long-term condition, including conditions affecting the heart, kidneys and metabolism.
The surgery has introduced a new approach to identifying, supporting and monitoring these patients, with each person receiving a care plan tailored to their needs. Patients can view their plan on paper or through the NHS App.

Healthcare professionals work together in small teams so patients can receive support from the right person without always needing to see a doctor. The surgery also works with specialist services at St George’s Hospital, giving patients access to expert advice while allowing them to receive most of their care closer to home.
Since the changes were introduced:
- the number of patients with well-controlled blood pressure rose from 84.2% to 89.7%
- kidney health testing more than doubled, from 32% to over 74%
- statin use rose from 63% to 88%
- prescribing of medicines that can protect the heart and kidneys increased around fourfold.
The surgery has also reported a 28% fall in doctor appointments, helping free up capacity for patients who need same-day care.
The HSJ Awards shortlist follows an Outstanding rating from the Care Quality Commission and a patient feedback score of 91.9%.
The winners of the 2026 HSJ Awards will be announced on 19 November.
